The Bridge Youth & Family Services

1. Raffle Ticket Purchase Price

• The purchase price for one (1) raffle ticket is $25.
• The purchase price for five (5) raffle tickets, which are purchased at the same time by one purchaser, is $100.
• The purchase price for twelve (12) raffle tickets, which are purchased at the same time by one purchaser, is $200.

Raffle tickets "purchased" by any means other than with cash (e.g., check or credit/debit card, internet transaction) is not accepted and shall not be binding on The Bridge until cash is actually received by The Bridge.

Checks or money orders must be made payable to "The Bridge Youth & Family Services.”

2. Sales and Purchases Deemed Completed in Palatine, Illinois

All sales and purchases of raffle tickets shall be deemed made at the offices of The Bridge, 721 S. Quentin Road, Palatine, IL.

3. Deadlines

All raffle ticket purchases must be completed, and in the possession of The Bridge Youth and Family Services or an authorized representative of The Bridge Youth and Family Services by 3:00 p.m. on Friday, April 20, 2012, EXCEPT raffle tickets may be purchased from an authorized representative of The Bridge Youth and Family Services at "A Night at Place de la Musique" up and until the time of the drawing on Saturday, April 21, 2012.

4. Purchases May Be Void in the Event Payment Method is Dishonored

If a purchaser's check or credit/debit card charge is dishonored, the raffle ticket purchased accordingly shall be void, and shall not be considered for a prize, unless the purchaser pays all check or credit card charges and fees, in cash, prior to the raffle.  If the raffle has occurred, and the purchaser fails to pay the charges and fees, then the purchaser shall forfeit and return the prize. In the event of a dishonor as described above, the purchaser will also pay an additional $20.00 administrative fee to The Bridge Youth and Family Services.

5. ALL SALES FINAL; NO REFUNDS

ALL SALES OF RAFFLE TICKETS ARE FINAL, AND THERE SHALL BE NO REFUNDS FOR ANY REASON.

6. Purchaser’s Must be 18 Years of Age or Older

Purchasers of raffle tickets must be 18 years of age or older on THE DATE OF PURCHASE.

7. Notification to Winners

In addition to public announcement of the winner at the drawing ceremony, the name of the winner will be posted at Office of The Bridge Youth and Family Services, and will be posted at www.bridgeyouth.org.

8. Three Prizes Only

There will be a random, blind drawing for three (3) prizes:

• The Grand Raffle prize is $5,000 cash

• The First Prize is $2,500 cash and

• The Second prize is $1,000 cash.

Winners will be determined by blind drawings selected from all qualified raffle tickets.

The drawings will occur between 8:30 p.m. and 10:30 p.m. on April 21, 2012, at "A Night at Place de la Musique" held at The Sanfilippo Estate in Barrington, Illinois.

The winner(s) need not be present to win.

The winner(s) will be notified as provided in Rule 7.

9. Name of Entrant Only Entitled to Prize for Winning Ticket

Each raffle ticket purchased and accepted as described above shall constitute an "entry" into the raffle, and the person to whom the raffle ticket is registered shall be the "entrant." In the event that more than one name is stated on the ticket registration form, the ticket shall be registered to the first name listed, who shall constitute the sole "entrant" for that ticket.

The Bridge accepts no responsibility for disbursing prizes to any other person listed on the ticket.  Payment to or delivery of any prize will be made only to the "entrant." The entrant must supply his/her social security number if he/she is a winner. It is the sole responsibility of the entrant to allocate a winning prize to other participants in case of a multiple entry registration, and the winning entrant shall defend and hold The Bridge Youth and Family Services harmless and indemnified from claims or disputes among multiple claimants.

10. Conflicts Resolved by The Board of Directors

Any controversy shall be resolved by the Board of Directors of The Bridge Youth and Family Services.  The decision of Board of Directors of The Bridge Youth and Family Services on all matters affecting the Raffle will be final.

11.Winners Must Claim Prize

In order to receive a prize, each Winner must contact Barbara Sipe at The Bridge Youth & Family Services within 14 days of the drawings on April 21, 2012 and claim their prize.
